How To Become A Nutritional Client Of Dr. Maile Pouls 1. Call the "Health Enhancement Center" office at Toll Free 1-866-333-7399 and leave your name, phone number, and a good time for a return call from Dr. Maile Pouls. Please speak clearly and slowly. 2. Please have available a list of your most significant health concerns and any brief questions you may have regarding Dr. Pouls clinical expertise. 3. If you do choose to become a patient , you will be sent a 'New Patient Packet', which includes an in-depth history intake form, health survey, nutritional magazine articles written by Dr. Pouls, educational information regarding enzymes, diet, nutritional therapy, and an audiotape of one of Dr. Pouls' public lectures. Also included is a laboratory urine test-kit and instructions on how to collect and send your sample. 4. Once you have completed your history form, and health survey, please mail it to Health Enhancement Center, 517 Penstock Dr., Grass Valley, CA 95945. Send the urine sample (next-day mail) to the lab (address is on the box provided). Instructions for collection of urine sample & requisition form are provided inside plastic bag in box. DO NOT send check or history forms to the lab. 5. Please call Health Enhancement Center at Toll Free 1-866-333-7399 the day you mail your history form and lab sample to schedule your first phone consultation to review your individual body chemistry profile and nutritional status.
